Who are we?
Team Hybrid Care is an interdisciplinary research team focused on rethinking how health care is delivered to people living with chronic illnesses. We lead the wider NovA (Novel Hybrid Care Approaches for Chronic Conditions) Network on novel hybrid care approaches for health systems.
Why hybrid care?
Hybrid care models have the potential to make health services more effective, flexible and timely — offering people living with chronic illnesses better support that may lead to improved quality of life and better health.
Our research focus
Our research focuses on how these models affect care organization, data and work flows, and the everyday work of health professionals. We are also interested in how hybrid care influences patients’ sense of control and involvement in managing their own health. By understanding these interactions, we aim to support the development of care systems that are more responsive to patients’ needs in a changing world.
